You’re exercising.


Maybe you’ve even cut calories, stopped eating late at night, and started walking more.

But the scale barely moves. Or worse—you keep gaining weight.

It’s frustrating. And after a while, it’s easy to think: “Maybe I just need more willpower.”

But weight regulation is more complicated than willpower or calories alone.

For some people, one piece of the puzzle may be insulin resistance.


Insulin resistance can make it harder for your body to manage blood sugar and may contribute to increased hunger, changes in energy, and weight gain over time.

And here's what makes it especially important:

Insulin resistance can develop years before blood sugar reaches the level used to diagnose type 2 diabetes.


Understanding what is happening earlier may give you an opportunity to make meaningful changes.


What Is Insulin Resistance?

Insulin is a hormone made by your pancreas.

One of its most important jobs is helping manage the glucose—or sugar—in your blood.

After you eat carbohydrates, your body breaks many of them down into glucose. That glucose enters your bloodstream.

Your pancreas responds by releasing insulin.

Think of insulin as a signal.

It tells cells throughout your body:

“Energy is available. Take in some of this glucose and use it or store it.”

When your cells respond normally to insulin, this system works remarkably well.

But sometimes the cells become less responsive to insulin's signal.

That's insulin resistance.

Your pancreas may compensate by producing more insulin to help keep your blood sugar within a normal range.

For a while, that compensation may work.

Your glucose can still look relatively normal while your body is producing more insulin to accomplish the same job.


Can Insulin Resistance Cause Weight Gain?

The relationship goes both ways.

Excess body fat—particularly visceral fat around the abdominal organs—is associated with insulin resistance.

But insulin resistance and higher insulin levels can also influence processes involved in hunger, energy storage, and how readily stored fat is released.

That does not mean insulin is the only thing controlling body weight.

At the same time, you may experience increased hunger, cravings, fatigue, or energy swings that make maintaining healthy habits harder.

Again, this doesn't happen exactly the same way in everyone.

What Are the Signs of Insulin Resistance?

This is where things get tricky.

Insulin resistance often doesn't cause obvious symptoms.

Some people may have insulin resistance for years without knowing it.

When signs or associated conditions are present, they may include:

  • Increasing waist circumference

  • Difficulty managing weight

  • Elevated triglycerides

  • Low HDL cholesterol

  • Elevated blood pressure

  • Higher fasting glucose

  • Prediabetes

  • Fatty liver disease

  • Polycystic ovary syndrome (PCOS)

  • Darkened, thickened skin in certain areas, known as acanthosis nigricans


Fatigue, hunger, or cravings can occur for many reasons and do not diagnose insulin resistance by themselves. That's an important distinction.


Can You Have Insulin Resistance With Normal Blood Sugar?

Yes.

This is one of the most important things to understand about insulin resistance.

Your body works hard to regulate blood glucose.

Early in the development of insulin resistance, the pancreas can sometimes compensate by producing more insulin.

That may allow glucose levels to remain within the normal range.

Over time, however, that compensation may become less effective. Blood glucose can begin to rise, potentially progressing toward prediabetes and eventually type 2 diabetes in some people. This is why metabolic health involves more than looking at a single glucose measurement.


What Tests Can Help Evaluate Metabolic Health?

There isn't one perfect test that tells the entire story.

Depending on your health history and individual situation, a healthcare provider may consider information such as:

  • Fasting glucose

  • Hemoglobin A1c

  • Lipid panel

  • Triglycerides

  • HDL cholesterol

  • Blood pressure

  • Waist circumference

  • Liver enzymes

  • Fasting insulin

The bigger point is this:

One number rarely tells the whole story.

What Can You Do About Insulin Resistance?

And many of the strategies that improve insulin sensitivity also improve overall health.


1. Build and Use Your Muscles

You don't need to become a bodybuilder.

Walking, resistance training, cycling, and other forms of physical activity help your muscles use glucose.

Both aerobic exercise and resistance training can improve insulin sensitivity.

2. Walk After Meals

Something as simple as taking a short walk after eating can help reduce the rise in blood glucose after a meal.

You don't need a brutal workout.

Consistency matters.

3. Eat More Minimally Processed Foods

A diet built around vegetables, fruit, quality protein sources, healthy fats, fiber-rich foods, and minimally processed carbohydrates can support metabolic health.

The goal isn't necessarily to eliminate carbohydrates.

It's to improve the overall quality and balance of your diet.

4. Prioritize Protein and Fiber

Protein and fiber can help with fullness and make meals more satisfying.

Fiber-rich foods may also help support better glucose regulation and gut health.

5. Strength Train

This deserves to be mentioned twice.

Maintaining muscle becomes increasingly important as we get older.

Resistance training can improve strength, preserve muscle mass, and improve insulin sensitivity.

6. Get Enough Sleep

If you're doing everything right with food and exercise but consistently sleeping five hours a night, you're missing an important piece of metabolic health.

7. Work Toward a Healthy Weight—Without Making Weight the Only Goal

For people carrying excess body fat, even modest weight loss can improve insulin sensitivity and metabolic markers. But the goal isn't simply making the number on the scale smaller.

We also care about: waist circumference, muscle mass, strength, blood pressure, blood sugar, triglycerides, sleep, energy, and long-term health.


What Does the Research Say?

Insulin resistance is a well-established feature of metabolic dysfunction and plays a central role in the development of type 2 diabetes. Research also shows that lifestyle changes can make a meaningful difference.


One of the most important studies was the Diabetes Prevention Program, a large randomized clinical trial involving people at high risk for developing type 2 diabetes.

An intensive lifestyle intervention focused on healthy eating, physical activity, and modest weight loss reduced the incidence of type 2 diabetes by 58% compared with placebo over an average follow-up of about 2.8 years.


Exercise is particularly important.

Research has consistently shown that both aerobic and resistance exercise can improve insulin sensitivity and glucose metabolism. And importantly, some benefits of physical activity can occur even before dramatic changes appear on the scale. That's why we don't view exercise simply as a way to “burn calories.” Movement changes how your body handles energy.
